We would like to offer you an initial health and wellness evaluation (a $175.00 value) at no cost to you. This offer enables you to meet with our clinic care coordinator to discuss your health history and goals concerning your whole life inclusive of mind, body, spirit and emotions. If we are a good fit for each other, our care coordinator will create a care plan for you and help you select the practitioners and treatment options that make sense for you. Dr. Wyatt’s Brief Intake Form:  for those patients just seeing Dr. Wyatt

Dr. Wyatt’s Woman’s Annual Exam Form:  for patients, even if you have been here before, who are getting an annual exam

Health and Wellness Exam/Multidepartment Intake:  for patients seeing multiple providers or coming for the free health and wellness visit

The Breast and Cervical Cancer Program: (free mam, pap and bloodwork for those who qualify) this program has its own special forms which will be given to you at the time of your first visit.

EAP Counseling:  this program has a special form which will be given to you at the time of your first visit.
